Please note, completion and submission of the form below does not guarantee a donation. Each charity is eligible to receive one donation per season. As multiple events often benefit the same charity, please note that a donation to your charity may have already been issued through another event. The Elks Community Department will contact you to advise of the status of your request. All donation requests must be submitted a minimum of 4 weeks prior to the event.
